EP申請に関して、現在は紙ベースの申請書を提出するのではなく、WEB上の「EPオンライン」というMOMのサイトからの申請に統一されています。
下記はEP申請の際に提出が求められている書類ですが、MOMのサイトからそのまま転記します。（MOMのサイト）

Documents required for Employment Pass
You need to submit documents such as copies of passport details and educational certificates along with the Employment Pass application.
You will need these documents to apply for an Employment Pass:
- Personal particulars page of the candidate’s passport.
- Company’s latest business profile or instant information registered with the Accounting and Corporate Regulatory Authority (ACRA).

上記を見ますと、現在EP申請に際て提出が求められているのは、
（１）ご自身のパスポートのお写真のページのコピー（PDFフォーマットで問題ないです。）
（２）採用企業の登記情報（これは採用企業が用意致します。）
（３）ご自身の卒業証書（英文）
の三点となっています。
ただ、「EPオンライン」のサイト上では、これまでの「職歴」について、
・在職期間
・会社名
・国（勤務国）
・職名／ポジションタイトル（セールスマネジャーなど）
・離職時のお給料額（シンガポールドル換算で）
などの情報入力が求められますので、あらかじめ「英文」でご用意頂くのが良いかと思います。
